Notes on completing the registration form

Editor's Note: You should have received a Registration Form on a separate white A4 sheet with this newsletter; if you need another for a colleague, please copy that one, or click here for another one. It is a good idea to make a copy of the completed form before sending it, in case it gets lost in the post, and bring it with you to registration.

Accompanying persons should complete the form for accommodation, meals, and any special and travel requirements. Mark it clearly Accompanying person and attatch the form to that of the person you are accompanying. Send them both in together.

Please complete this form legibly.

  1. Put the name you wish to appear on your conference badge
  2. The affiliation given here will appear with your name on the conference badge
  3. All information about this meeting will be sent to the address given here.
  4. Tick one of the eight status boxes
  5. If you are a student, please give the name of your supervisor and ask them to sign the form here.
  6. All late registrations will be charged £125 regardless of status.
  7. Add £12 if you are not a paid-up member of the BCA at the time of application.
    This payment can be converted into BCA membership by filling out a membership form at the meeting.
  8. Enter the appropriate registration fee.
  9. Circle the days you wish to attend the meeting and the sessions you hope to attend. Note that on Tuesday there are workshops for the CCG and the IG.
  10. Circle the nights and type of B & B accommodation required.
  11. Circle the evenings you require dinner, however only circle Wednesday, if you are not attending the Conference dinner. Lunches (3 course self service) are included in the registration fee.
  12. Please tick the box to indicate if you would like to attend the reception at the Playfair Library. It is free but numbers are needed to book coaches.
  13. Please enter the amount if you wish to attend the Conference dinner.
  14. Payment is by cheque only, payable to 'Heriot-Watt University'. Sorry, no credit cards.
    Please check that your cheque is signed and made out correctly.
  15. If you have any special requirements e.g dietary (vegetarian), wheelchair access, ground floor accommodation, etc....
    Provision cannot be made if this information is not given on the registration form.
  16. Please indicate whether you will be travelling by car and require a parking space.
  17. If you are coming by train and would appreciate a Conference mini-bus for which a small charge will be made, please give your estimated arrival time at which station. If there is sufficient demand bus times will be posted on this web site.
